Below are links to the four (4) forms which you will need to (a) download, (b) complete online, (c) print and then (d) sign.
All information should be TYPED where space allows - DO NOT print the form then handwrite details - handwritten forms will not be accepted.
Forms should be signed by the appropriate persons before submitting to the Dandenong Stingray's office.
Completed forms should be returned at the first pre-season training session.

AFL Victoria Player Selection Policy (to be read prior to signing the Selection Application Form)
The objectives of this Policy are:
(a) To clearly document and make available to all Players eligible to play in the TAC Cup, their parents and the member clubs of AFL Victoria, the process and criteria to be used by AFL Victoria and its member clubs in selecting Players for TAC Cup club training squads, Player lists and match teams to compete in the TAC Cup in the year 2017; and
(b) To select Players to compete in a high standard competition in the TAC Cup so as to enable them to develop to their full potential as a football Player and to equip them for a career in Australian Football.
